Legal & compliance
This sets out the plan management services Horizonte will provide under the NDIS, made under the NDIS Act 2013 and the NDIS rules, to support your choice and control.
Horizonte does not pay or receive payment for referrals. You may choose and change your providers and plan manager at any time.
Starts on signing; continues until your plan ends or either party gives [NOTICE PERIOD] written notice. You may leave anytime; we'll help transition your information.
We handle information under the Privacy Act 1988 (Cth). Concerns: [EMAIL] / [PHONE], or the NDIS Quality and Safeguards Commission on 1800 035 544.
Identity and contact details, your NDIS number and plan information, invoices and payment records, and communications.
Request access, correction or withdrawal of consent via [EMAIL]. You may complain to the OAIC.
Horizonte and its workers will act with respect for your rights and privacy; provide services safely and competently; act with integrity and transparency; promptly raise quality or safety concerns; and take reasonable steps to prevent and respond to violence, exploitation, neglect, abuse and discrimination.
