Getting paid is simple. No login needed to send an invoice.

Email your invoice as a PDF, or log in to upload it and track every step. Most correctly submitted invoices are processed within a few business days.

Plan Management Service Agreement

Between [COMPANY PTY LTD], ABN [ABN] ("Horizonte") and the Participant named in the onboarding form ("you"). Draft v0.1.

1. About this agreement

This sets out the plan management services Horizonte will provide under the NDIS, made under the NDIS Act 2013 and the NDIS rules, to support your choice and control.

2. What we will do

  • Receive, check and process provider invoices in line with NDIA price limits and your plan budgets.
  • Make claims from your NDIS funding and pay providers for approved supports.
  • Keep records and give you regular statements of spending and remaining funds.

3. What we will not do

  • We do not provide financial, legal or investment advice.
  • We do not pay for supports not funded in your plan or above NDIA price limits.
  • We do not charge you — our service is funded by the NDIS.

4. Conflict of interest

Horizonte does not pay or receive payment for referrals. You may choose and change your providers and plan manager at any time.

5. Term & ending

Starts on signing; continues until your plan ends or either party gives [NOTICE PERIOD] written notice. You may leave anytime; we'll help transition your information.

6. Privacy & complaints

We handle information under the Privacy Act 1988 (Cth). Concerns: [EMAIL] / [PHONE], or the NDIS Quality and Safeguards Commission on 1800 035 544.
